In short: Clinical interaction studies on shilajit do not exist. Because of its natural mineral content (including iron) and theoretical considerations, interactions are nevertheless conceivable – above all with blood thinners, levothyroxine, antidiabetic drugs and iron supplements. Anyone who takes medication regularly should therefore use shilajit only after consulting a doctor or pharmacist and keep appropriate timing gaps.

Why interactions are conceivable in the first place

Shilajit is a mineral-rich natural product. This is precisely where the most plausible points of contact with medication arise: minerals such as iron, calcium and magnesium can form poorly soluble compounds with certain active substances in the gut and reduce their absorption – an effect well known from conventional mineral supplements. Added to this are theoretical considerations from preclinical work that cannot be transferred to humans but do call for caution.

Two things are worth knowing here: first, side effects and interactions of food supplements – unlike those of medicinal products – are not systematically recorded. Second, the few human studies on shilajit generally excluded participants who took medication regularly. A lack of reports therefore does not mean a lack of risk.

The four groups in detail

Blood thinners: the highest level of caution

With anticoagulants such as phenprocoumon, the therapeutic corridor is narrow: even small shifts can push coagulation values out of the target range. For shilajit there are no human data proving such a shift – but none ruling it out either. With a medication whose incorrect dosing can have serious consequences, the burden of proof is effectively reversed: without explicit medical clearance and close INR monitoring, we advise against experimenting here. The same applies, by analogy, to modern direct oral anticoagulants, for which no routine monitoring takes place and an interaction would therefore go unnoticed for longer.

Levothyroxine: the most well-understood mechanism

The fact that minerals can interfere with the absorption of levothyroxine in the gut is not specific to shilajit: it has been demonstrated directly in humans for iron supplements (Campbell et al., 1992, Annals of Internal Medicine) and is well described for calcium. Since shilajit naturally contains both minerals, the four-hour gap here is not excessive caution but the logical application of an established rule. Anyone starting shilajit while taking levothyroxine should have their TSH level checked after six to eight weeks – that way, any relevant change is noticed early.

Antidiabetic drugs: measure more closely instead of guessing

The indications of blood sugar effects come from preclinical work and are not transferable to humans. Even so: anyone using insulin or sulfonylureas has a real risk of hypoglycaemia, and every additional variable belongs in the treatment conversation. In practice this means: start only after consultation, measure more frequently in the first weeks and document the values – that way you and your doctor have a data basis instead of a gut feeling.

Iron supplements: a simple matter of addition

This is not about an exotic mechanism but about arithmetic: a prescribed iron supplement plus the natural iron content of shilajit results in a higher total intake than your doctor planned for when prescribing. Disclose both so that the dose remains appropriate – and, if the combination continues for a longer period, have your ferritin level kept in view. Other groups where caution makes sense

  • Blood pressure medication: Additive effects are discussed, human data are lacking. Anyone taking blood pressure medication should clarify the start with their doctor and measure their blood pressure more frequently in the initial period.
  • Immunosuppressants: There are no safety data for people with a suppressed immune system or autoimmune diseases – here we advise against use without explicit medical clearance.
  • Uric-acid-lowering therapy (e.g. for gout): The data are unclear; clarify use with your doctor beforehand.
  • Any new long-term medication: Mention shilajit at every medical history interview – food supplements belong on the medication list.

The timing rule: 2–4 hours as a pragmatic baseline

For mineral supplements, the rule of thumb is to separate them in time from sensitive medications – 2 to 4 hours is common, and at least 4 hours for levothyroxine. Since shilajit naturally contains minerals, it is plausible to apply the same rule. An example daily routine: levothyroxine in the morning on an empty stomach, breakfast after 30 minutes, shilajit not until midday or the afternoon. Important: the gap is a precaution, not a guarantee. It only addresses interactions during absorption in the gut – not other possible mechanisms. That is why consulting your doctor remains the decisive step.

Checklist for the conversation with your doctor or pharmacist

  1. Bring a complete medication list – including other food supplements.
  2. Show the specific product: the label with ingredients and recommended intake, plus the certificate of analysis (COA) – available for each batch of our products at Lab Tests & Quality.

  4. Ask specifically about timing gaps relative to your medications.
  5. Clarify whether monitoring values make sense (e.g. INR, TSH, blood sugar, blood pressure, ferritin).

What the study situation offers – and what it does not

On the tolerability of purified shilajit in healthy adults, a few small studies exist: in one placebo-controlled study (N=63 evaluated, 2×250 mg daily, 90 days), no serious adverse events were reported (Pandit et al., 2016, Andrologia). A review (Stohs, 2014, Phytotherapy Research) describes purified shilajit as well tolerated at the doses studied. Studies that specifically examined interactions with medication in humans, however, do not exist. The data are not sufficient for authorised health claims – and they are also not sufficient to give the all-clear for people on long-term medication. Frequently asked questions about shilajit and medication

Shilajit and Marcumar – is that possible?

Only with explicit medical clearance and close monitoring of coagulation values. Since human data are lacking, particular restraint is warranted with anticoagulants.

How much of a gap do I need from levothyroxine?

At least 4 hours – just as recommended for iron and calcium supplements. Take levothyroxine in the morning on an empty stomach and shilajit no earlier than midday, and have your TSH level checked after a few weeks.

I take the contraceptive pill – is there anything I need to consider?

There are no specific data on hormonal contraceptives; no concrete mechanism is described in the literature. If you want to be on the safe side, ask at your pharmacy and keep the general timing gap.

Do I need to stop taking shilajit before surgery?

The usual recommendation is to stop taking food supplements one to two weeks before a planned procedure and to inform the anaesthesia team. Ask specifically during your pre-operative consultation.

Do these notes also apply to shilajit tablets?

Yes. Tablets contain the same constituents as the resin – for interactions, the dosage form makes no relevant difference.
